TG Oxygen, LLC DBA Fairway Medical (“Fairway Medical”, “we”, “our”, or “us”) is an Accredited Durable Medical Equipment (DME) Provider serving Arizona, committed to compliance, patient safety, and continuity of care. Below are our policies governing the use of our services, including our website, text messaging program, and DME services such as continuous positive airway pressure (CPAP) devices and oxygen therapy equipment. When does this Privacy Notice not apply?

This notice does not apply to other data processing by Fairway Medical or to third-party websites linked from our Platform. We encourage you to review third-party privacy policies.

We collect personal information that identifies or could be linked to you, such as identifiers (name, phone number, email) and sensitive data (location, payment details). We retain information based on legal requirements, litigation needs, statutes of limitations, and industry best practices, deleting it when no longer necessary.

  • Registering for the Platform: We collect name, phone number, email, mailing address, real-time location, age, and payment card details to set up accounts and communicate about the Platform.
  • Using the Platform: We collect identifiers, auditory/visual information (e.g., video calls), location, and messages to facilitate Platform use.
  • Requesting Information: We collect name, email, phone number, and request details to respond to inquiries.
  • Email Subscriptions: We collect email and contact preferences for newsletters, with an option to unsubscribe.
  • Website Chat: We collect name and disclosed information to provide customer service.
  • Text Subscriptions: We collect device ID and IP address to send texts about CPAP prescriptions and supplies.

Note: Provide only relevant information; avoid sharing sensitive data like Social Security numbers.

B. Information Collected Automatically

We use cookies, web beacons, and HTML5 local storage to collect device, usage, and location data to enhance user experience, conduct analytics, and prevent fraud. You can disable cookies, but this may limit functionality.

  • Google Analytics: Aggregated data for performance analysis (Google’s privacy policy).
  • Web Beacons: Track email and ad interactions in aggregate.

Terms of Service

Emergency Preparedness Plan
